The affidavit pretty much depends on your particular facts. There really is no standard form until someone develops a format for a particular situation.
For example:
If you are older than 21 years of age, and you are your own man, not incapacitated, a member of the body politic, what ever else you deem important and true about you in particular.
Then let's say you paid Tom 100 U.S.D. in single dollar coins for a pink automobile, that is marked with a number 1234567890xx99Q1 and uses gasoline and has four doors. Tom agree to sell it to you and you agreed to buy it, and both of you deem the trade done.
Now, you heard a rumor someone says that Tom feels cheated, you confront Tom and he states that he has not said anything like that, and again he states to you that he deems the deal done.
You affidavit may read something like this:
State of New Jersey ) ) S.S. Union County )
TO WHOM THESE PRESENT WRITINGS MAY COME:
Greetings,
I, Jeremyw, being of lawful age, and sound mind, a Citizen from the State of New Jersey by birth on the land, and the State of New Jersey being a State of the union of states styled the United States of America, and being competent to make this affidavit from my personal knowledge state the following:
1. On March 1, 2012 at 2:00 PM local tine, I, Jerwmyw, met with Tom Seller at Crossler Junction to purchase from him one (1) automobile described as follows: Make: Honda, Model: Accord, Color: Pink: Carriage Type: 4 Door Sedan, Engine: 4 Cylinder gasoline. Unique identification number: 1234567890xx99Q1.
2. Soon thereafter such time as stated above, Tom Seller and I agreed to consummate the sale and purchase of the above automobile for agreeable mutual valuable consideration. I paid Tom Seller, One Hundred Dollars, in minted coin, lawful money of the United States of America, and in exchange I received the above described automobile, to the mutual satisfaction of both parties.
3. Tom Seller stated and guaranteed to me, clean title to the said automobile.
4. Soon there after, Tom Seller and I agreed that our business had been done and concluded, and then parted ways.
Futhermore I, Jeremyw, affiant sayeth not.
